On-Site Visit/Mock Survey

 An On-Site Visit/Mock Survey will be scheduled during the initial application process; in order to set deadlines and ensure accountability on both the provider and Primas’ part. All On-Site Visit/Mock Surveys are conducted by a trained Primas Administrative Solutions consultant.

 On-SiteVisit/Mock Survey Process:

• Opening Conference
• Interview the leader/executives of the organization and a representative of the Board and/or Advisory Committee
• Review minutes from staff meetings, board meetings and planning sessions
• Review Budget and Accounting System
• Review contracts and interview the business manager/accounting clerk
• Review personnel records
• Review In-Service Records
• Review Client Records
• Implement Quality Improvement Program
• Review Compliance Program
• Home visits
• Interviews with staff
• Closing Conference (discuss deficiencies and corrective action)

Once On-Site Visit/Mock Survey has concluded you will receive a deficiency report via-email outlining the following:

• Requirement / Standard Observed
• Deficiency
• Recommendations and corrective action required
