      • Conducing Background checks on Selected Staff
      • Verification of Licensures
      • Policy implementation date
      • Application submission date
        • Prepare Civil Rights Package/855-A
        • Prepare Medicare Application
        • Submittal of Accreditation Application
        • OASIS Transmittal (TEST TRANSMITTAL)
        • Patient file set up
          • Patient files must meet State & Medicaid requirements
          • Quality Improvement Implementation
            • Every provider must have a Quality Improvement Program, to ensure compliance.
            • Quality Improvement must include Patient Care.
            • Compliance Program Implementation
              • A Compliance Program is mandatory for State regulations and must meet requirements
              • Agency On-Site Visit/Mock Survey
              • Accrediting Body conducts their on-site visit
              • Medicare Provider number is issued
