 Creating a PHC & Community Support Services (HCSS) Company must be a very methodical precise process.

 Primas Administrative Solutions has developed a very successful program; milestones which allows a provider to measure success every step of the way.

 The first step is to fill out your application with Primas Administrative Solutions. Filling out your Primas Administrative Solutions application is a method of setting up an account with our company and establishing your organization's needs.

 Primas Administrative Solutions will then determine a process which bests fits you and a structure for your PHC & Community Support Services (HCSS) Company so no step is over looked.

  • Establishing your company as a legal entity
    • Tax ID, Register with the State and form the articles of incorporation, DBA papers and/or establish a governing body. You can contact the Office of Secretary of State at 512.463.5555 to check if the name of your agency is available.
    • Select the Company's Location
    • Establishing your NPI number
    • Administrator & Alt. Administrator's  16 Hour Initial Credit Hours
    • Filling Out TXDADS Application
      • Conducing Background checks on Selected Staff
      • Verification of Licensures
      • Policy implementation date
      • Application submission date
        • Prepare Civil Rights Package/855-A
        • Prepare Medicaid Application
        • Patient file set up
          • Patient files must meet State & Medicaid requirements
          • Quality Improvement Implementation
            • Every provider must have a Quality Improvement Program, to ensure compliance.
            • Quality Improvement must include Patient Care.
            • Compliance Program Implementation
              • A Compliance Program is mandatory for State regulations and must meet requirements
              • Agency On-Site Visit/Mock Survey
              • State Survey conducts their on-site visit
              • Medicaid Provider number is issued (*must meet Medicaid State Regulation Contracts*)
